Truck driver runs over fellow employee

The facility has been cited for truck-related issues in the past.

A truck driver ran over a coworker at a grain cooperative in Minnesota on Tuesday. 

The accident happened on January 25th at around 2:40 p.m. at Sunrise Ag in Buckman.

According to Star Tribune, the grain truck, driven by 38-year-old Chad Pietrzak, backed over 37-year-old Nathan Rothstein, who was pronounced dead at the scene. 

Records reveal that the facility was investigated by OSHA back in February of 2010, during which numerous citations were issued. The violations found during that investigation include a lack of training on how to safely operate industrial trucks.
