Police in Indiana responded to a fatal incident at a Texas Roadhouse restaurant on Thursday.
The incident occurred shortly before 3 p.m. on August 8 in Warsaw, Indiana.
According to an initial investigation by the Warsaw Police Department (WPD), Lloyd Dale Howard, 77, was operating an empty grain hauler south on SR 15 through 350 North.
Witnesses reported that Howard was slumped over the wheel as the truck veered right, off the roadway, through a deep ditch, and over an access road.
The truck finally came to a stop when it collided with a Texas Roadhouse restaurant in a shopping plaza.

WPD says that the grain hauler also hit a NIPSCO electrical box prior to colliding with the restaurant.
Over a dozen employees were inside the restaurant, but no one was hurt.
First responders provided CPR at the scene.
Howard was transported to a nearby hospital, where he was later pronounced dead.
The incident is under investigation, but WPD told the Warsaw Times-Union that they believe that Howard suffered from a medical emergency prior to the crash.
