Out-of-service order issued after dual set of semi trailer wheels detached on Ontario highway

Police issued charges as well as an out-of-service order after a commercial vehicle driver lost a dual set of trailer wheels in Hamilton, Ontario, on Tuesday.

The incident occurred around 12:30 p.m. on December 30, 2025, on the Fort Erie-bound Queen Elizabeth Way (QEW), according to the Ontario Provincial Police Highway Safety Division.

Police said that a dual set of trailer wheels detached as a commercial vehicle was traveling on the QEW, but fortunately no other vehicles were struck, and no injuries were reported.

Following OPP’s investigation into the “Wheel-Off” incident, the trailer was placed out-of-service.

Additionally, OPP charged a 62 year old male from Pelham with Wheel Separation – Commercial Motor Vehicle and Fail to Accurately Complete Daily Inspection Report.

“The results of this incident could have been catastrophic. CMV operators have a responsibility to ensure their equipment is inspected and safe prior to use,” police said.
