OSHA investigating after man dies in attempt to load ‘metal spool’ onto flatbed truck with crane

Federal authorities are investigating a fatal workplace accident that took place at a facility in Illinois this week.

At noon on March 31, 2026, the Carol Stream Police Department (CSPD) responded to S & S International located in Carol Stream, Illinois, for a reported work place accident.

Police said that a 55 year old male employee attempted to load a “large metal spool” onto a flatbed truck with an overhead crane.

CSPD said that the crane “gave way” and that the spool fell from an elevated height and struck the employee.

Aid was rendered, but the employee did not survive.

Police say that the initial investigation does not show any indications of criminal activity.

The Occupational Safety and Health Administration (OSHA) has opened an investigation into the incident.

“The Carol Stream Police Department extends its deepest condolences to the family, friends, and coworkers affected by this tragic loss,” police said.
